Exercise Referral
The Up & Active Exercise Referral programme is a 12-week structured course of physical activity. Exercise options include chair-based exercise, group health walks, low level circuit, supervised gym, water-based activities and the Feel Good Gym. Each session has a charge of £3 during the 12 weeks. We ask a minimum attendance of one session a week.
To be eligible for this programme you must be at least 16 years old, inactive and have one or more health conditions. Conditions such as high blood pressure, diabetes, musculoskeletal problems or injuries, or mental health issues such as anxiety or depression.
This programme can be accessed through completing the online referral form, by telephoning 01282 477134 or a referral from a Health Professional, for example – GP, Practice Nurse, Physio.

This programme is suitable for anyone who wants to lose weight and understand more about the food they eat and how to develop a healthier relationship with it.
What to expect?
The sessions are delivered by trained advisors who will help you to focus on food choices, portion size and overall lifestyle habits. These sessions are in conducted in person and are supported by online resources and information that will give you the best chance of reaching your goals.
On this programme you will also be encouraged to try out lots of different activities that are tailored to your individual fitness level, local to you and that give you the greatest chance of success.

Community Cardiac Rehabilitation
A 12-week structured programme of progressive exercise and health education for people who have recently had a heart attack or heart surgery. Exercise options include group health walks, low level group activity, supervised gym, plus group health education. The cost of the programme is £36 for the 12 weeks.
This programme is only accessed via a referral from a Cardiac Nurse or Health Professional. Please make an appointment with your GP or health professional to discuss a referral.

Tea & Play (Play and Skills at Teatime Activities) and healthy family after school clubs in Burnley
This programme is for children and families living in Burnley and Padiham, focussing on children aged 5 to 8 years old. Children must bring a parent, carer or guardian to the sessions and siblings are welcome to attend.
The programmes will take place at primary schools and community venues across the borough on a rotating basis. Referred families will be offered the next appropriate programme.
